By completing this application I am giving Patriot Mortgage Company, Inc. permission to obtain my personal credit report. If you DO NOT want us to obtain a copy of your credit report upon application, please do not include your social security number and we will call you prior to "pulling" credit.

Please note that it is not necessary to complete all the credit information asked for in the application. We can fill those fields in after obtaining a credit report. Anything that you are not sure of, feel free to leave it blank and we will complete it together.
